Value transformation is about increasing the quality of care provided to the patient at a reduced price. It seems like a pretty simple equation, but it's drastically different from today's antiquated system that rewards uncoordinated, fragmented care. Instead of focusing their attention on caring for people who have fallen ill, health systems and payers are starting to shift their focus toward wellness and disease prevention. For example, instead of treating people after they develop diabetes, health care providers are working to identify pre-diabetics and help them avoid becoming diabetic through targeted care, education, diet and exercise. Population health will require organizations to have robust programs in place to help manage chronic diseases as well as provide comprehensive well-being, which in turn will result in a healthier environment.
